Create a network of Doctors (MDs/DOs/DNPs), Nurse Practitioners (NPs), Physician Assistants (PAs), Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), Certified Nursing Assistants (CNAs) and Caregivers actively involved in Senior care in the community and residential setting.
Assist our honored Seniors (older adult population) to attain or maintain a quality of life (QOL) at their level of functioning.
Assist individuals who need assistance with Activities of Daily Living (ADLs) to stay in a home-like environment and help minimize unnecessary hospital admissions/readmissions and/or trips to the Emergency Room (ER).
